When Symptoms Persist Despite Initial Treatment

I also commonly evaluate patients who have already completed an initial workup or treatment but continue to have symptoms, including those who:

  • Have completed physical therapy without adequate improvement

  • Have persistent pain despite medication or activity modification

  • Have already had X-rays, MRI, or other imaging

  • Have had a prior corticosteroid injection with incomplete or temporary relief

  • Want to explore PRP or other non-operative treatment options

  • Would benefit from a targeted ultrasound-guided injection or procedure

  • Are unsure whether orthopedic or surgical referral is the next appropriate step

The goal is to identify the source of the problem, review the available treatment options, and develop an individualized plan to help patients reduce pain, restore function, and return to the activities that matter to them.

A Complete Approach to Recovery

From the first evaluation through your return to activity, care is built around understanding the problem, defining your goals, and creating a practical path forward.

Restore Movement and Function.

Treatment begins with the most appropriate options for your condition and goals. Care may include Osteopathic Manual Treatment (OMT), functional rehabilitation, individualized home exercise programs, activity modification, mobility and strength training, medications, bracing, or other supportive treatments.

When beneficial, we collaborate with trusted physical therapists, athletic trainers, coaches, and other healthcare professionals to create a coordinated plan that addresses the underlying problem and builds a strong foundation for recovery.

Step 1

Listen. Evaluate. Understand.

Effective treatment begins with understanding the full picture. We take the time to review your symptoms, medical history, previous treatments, activity demands, and personal goals. A detailed physical and functional examination helps identify not only where you hurt, but why the problem may be occurring.

When additional information is needed, we may use musculoskeletal ultrasound, review prior studies, or coordinate X-rays, MRI, and other advanced imaging to establish a clear diagnosis and informed treatment plan.

Step 2

Return Stronger.

Recovery does not end when the pain improves. We continue working with you through rehabilitation, progressive loading, return-to-work, and return-to-sport planning to help restore confidence, durability, and long-term function.

When surgery or specialty treatment becomes the right next step, we help coordinate imaging, referrals, and postoperative recovery with trusted orthopedic surgeons and rehabilitation professionals. The goal is not simply to get you through an injury—it is to help you return to the activities, performance, and quality of life that matter most.

Step 3

Advance Recovery and Performance.

When symptoms persist or additional treatment is appropriate, more advanced options may be considered. These may include diagnostic musculoskeletal ultrasound, ultrasound-guided injections, corticosteroid treatment, hydrodissection, Platelet-Rich Plasma (PRP), and other orthobiologic procedures.

For athletes and active individuals, care may also incorporate recovery optimization, movement assessment, workload guidance, injury-risk reduction, and performance-focused strategies designed to support a safe and confident return to training and competition.

Injections & Ultrasound-Guided Procedures

When an injection or procedure is appropriate, I use a targeted, diagnosis-based approach rather than treating every painful joint or tendon the same way.

Many procedures can be performed with ultrasound guidance, allowing the medication or treatment to be placed precisely at the intended joint, tendon, bursa, or other structure. In many cases, evaluation, ultrasound, and treatment can all be completed during the same appointment.

Corticosteroid Injections

Corticosteroid injections can be helpful for reducing pain and inflammation in selected conditions, particularly when symptoms are limiting activity or interfering with rehabilitation.

Common uses may include:

  • Joint arthritis and inflammation

  • Subacromial bursitis

  • Greater trochanteric bursitis

  • Selected tendon sheath conditions

  • Other inflammatory musculoskeletal conditions

Corticosteroid injections can provide significant symptom relief, but they are not the best option for every condition or for repeated long-term use. I consider the diagnosis, prior injections, activity level, and treatment goals before recommending them.

Platelet-Rich Plasma (PRP)

PRP is an orthobiologic treatment prepared from a small sample of your own blood. The blood is processed to concentrate platelets and plasma, which are then injected into the targeted area, typically using ultrasound guidance.

PRP may be considered for selected conditions such as:

  • Knee osteoarthritis

  • Tennis or golfer's elbow

  • Patellar tendinopathy

  • Achilles tendinopathy

  • Gluteal tendinopathy

  • Selected rotator cuff conditions

  • Plantar fascia disorders

  • Selected muscle, tendon, and ligament injuries

PRP is not appropriate for every injury. I use it selectively based on the diagnosis, available evidence, previous treatment, and the patient's goals.

For appropriate patients, consultation, ultrasound evaluation, and PRP treatment may be completed during the same visit.

Hyaluronic Acid / Viscosupplementation

Hyaluronic acid injections may be an option for selected patients with knee osteoarthritis, particularly when symptoms continue despite exercise, physical therapy, medication, or other conservative treatment.

These injections are designed to improve the environment within the joint and may provide another non-operative option for patients trying to remain active and delay or avoid surgery when appropriate.

Coverage varies by insurance plan, and treatment recommendations are individualized.

Diagnostic Injections

Sometimes the most important question is determining where the pain is actually coming from.

A targeted injection of local anesthetic can help identify whether a particular joint or structure is responsible for the patient's symptoms.

Diagnostic injections can be particularly useful for complex hip, shoulder, or other musculoskeletal pain when imaging shows multiple possible sources.

Joint Aspiration

When a joint becomes significantly swollen, fluid can sometimes be removed using a needle, often under ultrasound guidance.

Aspiration may:

  • Reduce pressure and discomfort

  • Allow fluid to be evaluated when clinically necessary

  • Help guide further treatment

Common examples include swollen knees and selected cysts or fluid collections.

Tendon & Soft-Tissue Procedures

Ultrasound guidance also allows treatment to be directed precisely around tendons, tendon sheaths, bursae, fascia, and other soft-tissue structures.

Depending on the condition, options may include:

  • Targeted tendon or peritendinous injections

  • Tendon sheath injections

  • PRP for selected chronic tendon disorders

  • Bursa injections

  • Plantar fascia procedures

  • Selected cyst or fluid aspiration

  • Other ultrasound-guided soft-tissue procedures

Nerve Hydrodissection

For selected nerve entrapment conditions, ultrasound can be used to identify the nerve and guide fluid around it to help separate it from surrounding tissues.

Hydrodissection may be considered in certain peripheral nerve conditions when symptoms and ultrasound findings support the diagnosis.

Choosing the Right Treatment

There is no single injection that is best for every musculoskeletal problem.

Some patients benefit from a corticosteroid injection. Others may be better candidates for PRP, viscosupplementation, rehabilitation, OMT, or no injection at all.

My goal is to first identify the underlying problem and then recommend the treatment that makes the most sense for the diagnosis, activity level, and long-term goals.

Insurance-based and cash-pay options may be available depending on the service. PRP and certain elective procedures are commonly offered on a cash-pay basis.
